The Federal Aviation Administration issued an “extreme risk” warning after a SpaceX Starship test flight exploded less than ten minutes after launch in January, placing three commercial passenger jets in danger over the Caribbean. FAA documents reveal that debris scattered across controlled airspace for nearly an hour, forcing pilots to reroute, declare fuel emergencies, and fly through a temporary no-fly zone. One JetBlue flight to San Juan was told it could proceed only “at your own risk,” prompting a Mayday emergency declaration. Air traffic controllers intervened to prevent midair conflict after two aircraft flew dangerously close. The FAA also stated SpaceX failed to promptly notify controllers via the required hotline. All 450 passengers landed safely.
